1. Scope of Services
Anemara Wellness Practice LTD provides private general practice services, including online consultations, home visits, health assessments, chronic disease management and related healthcare services. By booking an appointment, you agree to these Terms & Conditions.
2. Appointments
Appointments are available by prior booking only. Appointment times are reserved specifically for you. We ask that you attend on time and provide accurate information when booking. We reserve the right to reschedule appointments due to clinical emergencies, illness or circumstances beyond our reasonable control.
3. Telemedicine
Online consultations are suitable for many conditions but may not be appropriate for every medical concern. The doctor may recommend an in-person assessment, attendance at an urgent care service, Emergency Department review or referral to another healthcare professional. Clinical decisions remain at the discretion of the doctor.
4. Home Visits
Home visits are available in selected areas and subject to availability. Patients must ensure that the doctor can safely access the property and conduct the consultation in a safe environment. Home visit fees will be confirmed before booking.
5. Fees and Payment
Consultation fees are payable at the time of booking unless otherwise agreed. Additional charges may apply for blood tests, ECGs, laboratory fees, medical certificates, procedures, consumables, specialist investigations and third-party services. Any additional cost will be explained before proceeding whenever possible.
6. Prescriptions
Prescriptions are issued only where clinically appropriate. The doctor is not obliged to prescribe medication requested by a patient. Certain medications may require further assessment, monitoring or specialist review.
7. Certificates, Reports and Forms
Medical reports, insurance forms, employment forms, letters and certificates may incur additional fees. Completion times may vary depending on complexity and workload.
8. Emergency Care
Anemara Wellness Practice LTD is not an emergency medical service. If you are experiencing a medical emergency, call 112 or 999 or attend the nearest Emergency Department.
9. Patient Responsibilities
Patients are responsible for providing accurate information, informing us of relevant medical history, following medical advice where appropriate, and seeking urgent medical attention when advised.
10. Limitation of Service
Healthcare outcomes cannot be guaranteed. Medical advice is based on the information available at the time of consultation.
11. Appointment Cancellation Policy
Cancellation, rescheduling, late arrival, missed appointment, teleconsultation and home-visit charges are governed by our Appointment Cancellation Policy. That policy forms part of these Terms & Conditions.
12. Privacy
Personal information is handled in accordance with our Privacy Policy and applicable data protection legislation.
13. Changes
We may update these Terms & Conditions from time to time. The latest version published on our website will apply.
